Incident Report Form

This form allows victims, witnesses, or complainants to report the details of an incident through an online submission.

Parking Permit Application

Apply for a vehicle or motorcycle parking permit to park on campus. Have your license, insurance, and vehicle info ready.

Vehicle Storage Request

Request to store your personal vehicle on campus with an understanding that BYU–Hawaii is not liable for any damage or loss. Completion of this form confirms your acceptance of all storage terms and conditions.

Aloha Late Night Shuttle Sign-up Form

Campus Security provides a Late Night Shuttle Service for all students free of charge. There are 2 shuttle times; one at 10:00 pm and the other is at 12:00 am.  Please write your home address where you’ll be dropped off at. We will transport students from campus to their off-campus homes in Laie, Hauula, Punaluu, and Kahuku.
